Forms and Document Information
To fill out and return forms electronically, right click on the form and click on “save as” to download to your computer. When you open the document on your computer in Adobe, click on the “tools” tab. Select “fill and sign”. This allows you to type in the fields. Make sure to “save as” when you are done, leaving the name of the document and adding an underscore and your students initials at the end.
Start of School Forms

Please fill out if you think your student has a disability, or if you have documentation that your student is disabled.
