Forms and Document Information

Complete Electronically

To fill out and return forms electronically, right click on the form and click on “save as” to download to your computer. When you open the document on your computer in Adobe, click on the “tools” tab.  Select “fill and sign”.  This allows you to type in the fields. Make sure to “save as” when you are done, leaving the name of the document and adding an underscore and your students initials at the end.

Start of School Forms

Visit the district Start of School Forms page for  Emergency Information and Student Release Form  in English,  FERPA & Opt Out form in English,  Survey to Identify Disabled Students  in English. Please fill out if you think your student has a disability, or if you have documentation that your student is disabled. The  McKinney-Vento (MKV) Program  provides services and support to students experiencing homelessness. Please fill out if you are experiencing housing instability.  Indian Student Eligibility Certification Form for New Students Only

Forms are Translated Forms: Amharic, Chinese, Oromo, Somali, Spanish, Tagalog and Vietnamese

These include information on Advanced Learning Opportunities (ALO) for K-8, Protection of Pupil Rights Amendment and information on childcare and free and reduced lunch.
