    To fill out and return forms electronically, right click on the form and click on "save as" to download to your computer.

    When you open the document on your computer in Adobe, click on the "tools" tab. Click on "fill and sign". 

    This allows you to type in the fields. Make sure to "save as" when you are done, leaving the name of the document and adding an underscore and your students initials at the end (_xx) (example:  504-2survey_identify_disabled_students_english2017 would be saved as 504-2survey_identify_disabled_students_english2017_ML)

These include information on Advanced Learning Opportunities (ALO) for K-8, Protection of Pupil Rights Amendment and information on childcare and free and reduced lunch.
